Bridge to Independence

The Bridge to Independence program is Mississippi’s Money Follows the Person (MFP) initiative.  A federal grant funded by the U.S. Department of Health and Human Services and the Centers for Medicare and Medicaid Services (CMS).  MFP is a 6-year demonstration grant funded by CMS in partnership with the Mississippi Division of Medicaid.

Golden Triangle Planning and Development District is a Mississippi Division of Medicaid Bridge to Independence provider.  Our trained community navigator will help residents of assisted living plan for a transition from a facility to a home in the community.

We provide the following Bridge to Independence services to older adults (65+) and persons with physical disabilities.

The following services are provided for 65 days after transition from an assisted living facility:

  • Transportation
  • Crisis/safety planning
  • Life skills training
  • Caregiver support (peer-to-peer and individual therapy)
  • Peer support
  • Transition care management
  • Attendant care

We also provide one-time financial assistance in the following areas:

  • Security and utility deposits
  • Household furnishings and goods
  • Moving expenses
  • Environmental accessibility adaptation
